Chrill Visiting Nurses bring the health care team to you, preventing hospitalization and aiding in recuperation from illness or injury.

Working in conjunction with your personal physician, a specially trained registered nurse develops an individualized plan of care to meet your specific health care needs and coordinates your care with other members of the health care team:

Rehabilitation Therapists

Medical Social Worker

Nutritionist

Certified Home Health Aide

The RN maintains regular contact with your physician regarding your health progress and provides:

bullet Blood pressure monitoring
bullet Diabetes tracking/monitoring
bullet Wound care
bullet Administering injections
bullet Instruction in proper use of medications
bullet Nutritional consultation
bullet Disease prevention techniques and/or treatment
